You have to complete all of the Provisioner quests for both cooking and brewing (last brewing one needs 14 Frostberries that are gated behind horid lvl 25 mobs and the cold zone) - once they are all done prov trainer then gives you the other quests in the chain to get the Pan

There is one specific bear that needs to die which is the place holder for this to spawn. If you want to know which it is you have to follow Khibera around and this will reflect the bear that is the Ph because it follows the same path
